Disability Assessment

Based on the scores of the Brigance and WISC-IV tests given to Tony (see attached documents), it is the determination of the school district that Tony is not eligible for special education services.  He will instead be considered socially maladjusted and will be given the appropriate accommodations as set up in a Section 504 Plan.

Section 504 Plan for _______________

School ________________

School Year ____________

Background

Tony Brennan is an 8 year-old boy who is new to the Typical School District this year. He is currently in the third grade; however, Tony is socially maladjusted and has had recent problems appropriately responding to others, both in and out of the classroom. Tony is currently at or above the standard competency for reading comprehension, math problem solving, number recognition, word recognition, and math computation at the third grade level according to the Brigance Inventory of Basic Skills.

Tony has only been in the Typical School District for four weeks. At first he was very reserved and showed no major signs of misbehavior. Recently however, Tony had been disrupting the classroom consistently and intentionally tried to start fights with other students, while even going to the extreme of kicking another student in the head. School administrators have tried numerous times to contact his mother and father but have been unsuccessful. Tony recently revealed to school personnel that he is no longer living with his parents and is currently living with his aunt, uncle, and three cousins. Tony was recently found guilty of stealing money from the purse of a classmate. Upon searching his locker for the stolen money, a hunting knife was found. Because of school and state-wide policy, the decision has been made to suspend Tony from school grounds for 180 days.

Section 504 Plan for Tony Brennan

Plan

Tony Brennan has been expelled from Typical Public Schools and will be unable to attend classes in any school building.  The district will instead hire a paraprofessional to instruct Tony on lessons developed by his general education teacher. Tony will be required to meet with this paraprofessional three times a week, Monday, Wednesday, and Friday, between the hours of 9 a.m. and 12 p.m. at the public library. In addition to Tony’s meeting with the paraprofessional, Tony will also meet once a week with the school social worker for at least a half hour at a location to be determined by the social worker and Tony’s guardian.  The school district will provide texts for Tony’s instruction in Mathematics, Science, Social Studies, and Language Arts.  Tony will also be actively involved in the development of a behavior intervention plan so as to better prepare Tony for reentrance into the general education population.

Goals

Several goals have been set for Tony Brennan that are to be successfully met at the end of his expulsion, the end of the 2004/2005 academic year.  One of these goals is for Tony to have learned and applied a new understanding of the types of behaviors that are appropriate for a classroom setting and in the presence of an authority figure.  In order to meet this goal, Tony will be involved in the adaptation and implementation of a behavior intervention plan.  Tony will also be continuing his general education curriculum throughout the time he is expelled from Typical Public Schools.  This continuance of his education will ensure that when he returns the following year he has not fallen behind the rest of his class and his test scores have stayed consistent or improved.  We also hope to be able to fully integrate Tony back into the general school population, without incident, at the beginning of the 2005/2006 school year.  It has been recommended that during the time he is absent from a normal school setting he participate in a recreational sporting team or other group activity that is of interest to him.  These activities are to be done outside of those that the school provides.  This will allow for his continued growth in his skills of relating with others in a social setting, such as school or a team activity.  The area of social relations is one which we all feel Tony needs to work to improve so things like this do not happen in the future.  It is also suggested that Tony work with the school psychologist to work through some of anger and home related problems which may have fueled these activities.  Each of these goals are to be met by the conclusion of the 2004/2005 school year in hopes that Tony will be fully prepared and excited about rejoining the other students at the beginning of the next academic year.
